Vaccine Pre-Sign Up Questionnaire

Please fill out this questionnaire to learn about your options to receive a COVID-19 vaccine in Thornton, or go here to read more: Thornton COVID-19 Vaccine

Per CDC Clinical Guidelines, given the lack of data on the safety and efficacy of mRNA COVID-19 vaccines administered simultaneously with other vaccines, the vaccine series should routinely be administered alone, with a minimum interval of 14 days before or after administration with any other vaccine. Please check back after your 14 days have passed to schedule a vaccination.
The CDC recommends that people with a history of anaphylactic reaction to any other vaccine, a previous dose of an mRNA COVID-19 vaccine, or its components should not receive mRNA COVID-19 vaccination (Pfizer-BioNTech or Moderna) unless they are under observation in a setting with advanced medical care available. Thornton Fire Department partners with North Suburban Medical Center vaccination clinics and connect you for assistance with scheduling a vaccination. If interested in assistance scheduling please email and provide the following information: Name, Email Address, Phone Number, Date of Birth – Note, you must be within the current vaccination phase of 60+.
